SALT LAKE CITY — Terri Jean Cook, 66, passed away Oct. 30, 2012.

Cook was born June 22, 1946 in Tooele to Eugene and Dorothy Lohnes. She married Delbert Allen (John) Cook on May 24, 1969. She was given the Commander’s Award for Civilian Service for hard work, dedication and devotion to all those she assisted. She was a member of St. Marguerite Catholic Church.

Cook is survived by her husband, John Cook; daughters Amy Miller (Matt), Jami Hall (Dustin); sister Susan Lohnes; grandchildren, nieces and nephews.

Cook was preceded in death by her brother, Craig Lohnes; sister-in-law Janiel Lohnes; mother and father.

A funeral Mass was celebrated at Saint Marguerite Catholic Church.
