SALT LAKE CITY — Zachary Paul Boehme, 37, passed away Oct. 31, 2011.

Boehme was born on April 23, 1974 in Salt Lake City to Brian and Michelle Boehme.

Boehme is survived by his children: Haylee, Nathan, Ethan, Kyrra; parents Brian and Michelle; brother Christian (Marjorie); sister Bethany (Jake); grandfather Frank Burleson; nephews and a niece.

Boehme was preceded in death by his grandmother, Aunt "GoGo," and great-grandmother Margaret.

A funeral Mass was celebrated at Saint Francis Xavier Catholic Church.
